The effect of telmisartan and rosiglitozane on metabolic parameters and blood pressure in type 2 diabetic hypertensive patients

    Abstract: Objective  To study the effect of telmisartan and rosiglitozane on metabolic parameters and blood pressure in type 2 diabetic hypertensive patients. Method  Forty-eight diabetic hypertensive patients received rosiglitozane (RSG) 4mg + telmisartan (Telm) 80mg, and without any other antihypertensive drugs.Results  After 6 months" treatment, the patients showed a significant reduction of mean blood pressure, and a positive influence insulin resistance.Conclusion  In type 2 diabetic hypertensive patients, the association of  rosiglitozane and telmisartan is suitable for the metabolic syndrome.

    糖尿病患者高血压的患病率随年龄而增加,由于糖尿病高血压对血管病变的影响，助长了患者发生视网膜病变、脑血管病变、冠心病、肾硬化和神经病变等的危险性，所以合理的降压治疗对此类患者尤为重要。本文研究了替米沙坦与罗格列酮联合用药对患者血压及代谢指标的影响，以及对胰岛素抵抗改善的作用。

1  资料与方法 

1.1  病例纳入标准：糖尿病诊断符合 WHO 1999 年诊断标准 ,应激性血糖增高及糖调节受损剔除。选择HbA1c>7%的糖尿病合并高血压患者48例，既往从未接受降压药物治疗，平均动脉压>115mm Hg,糖尿病史4～18年(7. 8±4.6）年 。

1.2  方法：每日给予罗格列酮4mg+替米沙坦80 mg口服，治疗6个月。治疗前后测定空腹血糖、血胆固醇、三酰甘油、胰岛素和脂联素的水平。胰岛素抵抗的程度用HOMA指数［FPI(U/ml)×FPG(mmol/L)/22.5］。血浆三酰甘油和胆固醇的水平测定采用氧化酶法，血糖使用葡萄糖氧化酶法。血浆胰岛素使用RIA法，糖化血红蛋白采用光色吸收法。脂联素采用取空腹血样本，采用ELISA法检测血清脂联素水平，敏感度为0.1mg/L。

1.3  统计学处理：所有数据采用均数±标准差（x±s）表示。脂联素、三酰甘油、胆固醇和胰岛素水平采用对数表示以获正态分布进行统计分析。计数资料用ANOVA方法分析，用药前后自身对照采用配对t检验。以 P＜0.05作为统计学标准。
