SURVEILLANCE REPORT

Subject: **************

0820 hrs.  Received call from Tom *****  that Subject had called in sick to ******* School System.

 Conducted the following surveillance pertaining to the activities of Subject *******

0915 hrs.    Checked the Subject’s residence at #*** Park Ave., *****, NY.  Subject’s vehicle, a  Black, Chevr. TrailBlazer was not observed.  Present in the driveway was a small sedan, NY reg. *******.  Parked on the street was a blue Dodge pickup truck, ******** and a Green Toyota sedan, *******.

    Checked the down town business area along Halstead Ave. East and West of Harrison Ave. Checked around the Firehouse on Harrison Ave. north of Park Ave.Checked the area North of the railroad tracks and near Town Hall.All with negative results.

1045 hrs.  Commenced surveillance at the center of town on Halstead Ave.

1130 hrs.  Checked the area *****, A/K/A ***** with negative results.

1210 hrs. Commenced surv. on Harrison Ave. and Park Ave.

1235 hrs.  Subject, driving a Blk Trail Blazer, NY reg. ******* adorned  with a yellow ribbon on the left side of the tailgate,  arrived at # *******.  Subject parked in the driveway, but partially on the sidewalk, walked up the steps, checked the mail in the mail box and entered his front door.  Subject was wearing Lt. gray pants, a dark shirt and a sweater vest, is clean shaven and has long gray hair.  Subject is extremely obese and appeared to climb the steps with difficulty.   **   (video taken)

1240 hrs.   Commenced surveillance ***** Ave. 

~1304 hrs.  Subject exited and then reentered the front door                   **   (video taken)

1306 hrs.  A female entered the front door held open by the Subject.        **   (video taken)

1319    hrs.  Subject opened the front door then closed it again.                  **  (video taken)

1321  hrs.  Subject exited the front door, entered his vehicle and departed.  ** (video taken)

 Subject drove up to ****** and proceeded west to the village of ***** where he parked at a CITGO gas station on Fenimore Ave. near Lafayette.

 **These videos were taken in reverse through the side view mirror of the surveillance vehicle.

1334     hrs.  Subject departed the CITGO station and drove back to *******

1341     hrs.  Subject pulled into the Vail Chevrolet Auto Dealership 195 Halstead Ave. and entered unobserved. Workmen were later observed looking in the rear of the vehicle. Lost sight of Subject .

1415 hrs  A check of the service area revealed that the Subject had departed Vail, unobserved.

1418 hrs.   The Subject’s vehicle was observed parked on the sidewalk next to the Firehouse  on ***** Ave. just north of Park Ave.

~1420 hrs.  Vehicle had departed unobserved.  Checked the subject’s residence and surrounding area with negative results.

1437 hrs.  Subject’s vehicle was again parked on the sidewalk at the Firehouse.  Other vehicles parked behind the Firehouse “Reserved for Firemen”, were ********.

1530 hrs.  Subject exited the Firehouse from a side door, entered his vehicle and departed.(video taken)

Subject drove very slowly, followed by a white van, up Park Ave. and across Halstead Ave. and across the railroad tracks.  The surveillance was broken off for security reasons, as the white van had appeared behind my vehicle.

The Subject’s residence and the Firehouse were checked with negative results.

1600 hrs.  Surveillance Discontinued.
